Create Your Order Form

This is a simple day for you. You need to create the order form people will use to sign up for your teleseminar, or order CDs or audio cassettes. This is the form that you’re going to use on the web and on the phone. At the very least, you want to record:
- Name
- Company name
- Address
- Phone
- E-mail
- Credit card number
- Address of the credit card statement (If there’s a mismatch between the billing address and the address statement address, the card might be declined.)
- How did you hear about us? This is useful in marketing and for paying commissions, if any.
- Special instructions from the customer
- Special instructions to the customer (such as "Your statement will read Parent Company Name.")
You also might ask people demographic *uestions, (such as age, sex) occupational questions (job title, buying power) or other *uestions that will help you market your products and services more effectively.
You can specify which information MUST be filled out and which is optional. Most shopping cart order forms will let you specify this information. For example, you might not need their fax number, but you definitely want their email address so you can contact them.
Finally, your order form should include information in the "header" at the top of the form that restates the offer. This is a standard marketing tactic for print, and it should be used for online forms as well.
The message can say:
"Yes, I want to become a better speaker! Please register me for this teleseminar."
You can also explain any procedures that need to be done, such as:
"Please specify whether you would like the audio cassette or CD version of this seminar.”
You also can insert alternative ordering methods, such as:
"If you have problems with this form, or would like to order by phone, you can call our friendly staff at xxx-xxx-xxxx."
You should copy the online order form and give it to your telephone staff or answering service so they can gather the same information, in the same order as your online system. This mirroring will save you lots of time by keeping the information in a standard format.

Note: If you are selling products like audio cassette recording or CDs of the session, you will need to create fields for those products, as well as line items for shipping and sales tax.
